We go to a lot of effort to document everything in our source code, even private methods and variables. We don't claim 100% coverage, but we're close and getting closer. Our source code is also consistent. We make sure that all the indentation is correct (hey, you might not like our style, but at least we stick to it!). You should be able to see the flow of logic at a glance.

We make our variable names names and method names meaningful. Sure they might be a bit longer than necessary, but that always helps understanding. We use a consistent naming system (no, it's not Hungarian, but we do use a few prefixes), so that you can easily tell what the intended use of a variable is.

Finally, we are obsessive about whitespace. It's just one of those things that you don't see (ha ha), but that makes a big difference to accessibility. Nicely spaced code, with things that go together logically placed close to each other, and things that don't separated out, makes for easy-to-understand code. It's something we think about so that you don't have to think about it, so that the code is just obvious.
